This is probably obvious to you guys, but I don't know it.  Say for example you have a house with an enemy squad in it.  For some reason or another, one of your squads has entered.  It's a pretty large house, and you got the drop on them.  Should you hit home with a "Fast Move" into that squad to start hand to hand, or should you instantly tell them to stop and shoot?  I see my squads throw a LOT of grenades regardless of the option I give them, and it -appears- as if they sometimes blow themselves up.

That would depend entirely on the type of squad you have entered the building with.
There are specialist close quarter equiped squads, have a look at the weapons make up of certain groups. ie; assault/pioneer and some others , many smgs indicate abillity for close assault as does experience (veteran squad). But allways as a rule of thumb rely on numbers, you need more men then your opponent to assault a defensive position.

Smoke and Suppression can also help.  Not all squads have smoke but if they do, try and use it.

As you have seen, if you assault, your squad will use grenades etc., but if you can also have the enemy targetted by another Team/Unit from another direction, this can reduce their capabilities.  A combination of smoke, suppression, and assaulting from 2 or more directions simultaneously is possibly the best way to succeed.

But if you stumble across the enemy in a building unexpectedly, then you may not have time to do these things, and have to make a quick call.  If you haven't got superiority in numbers/weaponry, then the odds are with the defenders, unless the enemy team is well degraded.  It may then be better to err on the side of caution and withdraw and wait for support to arrive.

Do soldiers blow themselves up and can they suffer from friendly fire?  Yes, it can happen....
